Founded in 1975 and based in Van Wert Ohio, Community Health Professionals (CHP) is a community based , nonprofit home health and hospice organization.  CHP delivers skilled nursing, in-home and inpatient hospice, private duty, therapy, adult day care and related services.  In-Home & Inpatient Hospice offers holistic care that respects the patients priorities and personal choices and provides spiritual and emotional support for the whole family.
